Lancet Oncology December 2006 pages 1017-1026 Review Diet, exercise, AND complementary therapies after primary treatment for cancer Dr Lee W Jones PhDa, , AND Wendy Demark-Wahnefried PhDa, b aDepartment of Surgery, Duke University Medical Center, Durham, NC, USA bSchool of Nursing, Duke University Medical Center, Durham, NC, USA Available online 27 November 2006. Summary Every year, more than 10 million people are diagnosed with cancer worldwide. In view of the substantial improvements in early detection AND treatment, even more patients can expect to be alive 5 years after diagnosis. With improvements in longevity, the late-occurring adverse effects of cancer AND its treatment are becoming increasingly apparent. Healthy lifestyle behaviours that encompass regular exercise, weight control, healthy nutrition, AND some complementary practices—eg, support groups, imagery—have the potential to greatly reduce cancer-treatment-associated morbidity AND mortality in cancer survivors AND can enhance quality of life. Here, we aim to review the strength of evidence for recommendations for exercise, weight management, nutritional practices, AND related complementary therapies; assess the perceived needs of cancer survivors for health information AND how they can access this information; AND discuss the resources available to oncology care providers AND patients about healthy lifestyle behaviours. Overall, this review provides important information to oncology care providers who counsel their patients on preventive lifestyle practices to maximise health AND longevity after a diagnosis of cancer. Complementary AND alternative medicine Use of complementary AND alternative medicine is prevalent in the general population AND in cancer survivors.8 Common categories of complementary AND alternative medicine include specific dietary OR exercise regimens (eg, Reiki, yoga), dietary AND herbal supplements, acupuncture, massage, AND psychological OR mind–body therapies (eg, imagery, journalling, support groups). Hann AND colleagues63 suggest that up to two-thirds of adult cancer survivors use some type of complementary OR alternative therapy, with 69% believing that it will prevent recurrence, AND 25% stating that it will offer a cure. A review by Monti AND Yang64 reports that use of complementary AND alternative medicine fulfils psychosocial needs that are inadequately addressed by the conventional biomedical system, yet survivors might derive potential benefit for managing side-effects, as well as reducing pain AND anxiety. Therefore physicians are encouraged to recommend selected complementary AND alternative therapies, such as support groups, massage, AND relaxation therapy that seem safe for cancer survivors. However, in view of the paucity of evidence on the biological agents (eg, chelation treatment, restrictive dietary regimens, OR dietary supplements), current guidelines do not endorse the use of these products OR regimens.11 Instead, patients are encouraged to access reliable sources of information, such as the US National Center for Complementary AND Alternative Medicine (table 3). Open dialogues with physicians AND other healthcare providers play an integral part in this process, especially given the potential for deleterious interactions between various complementary AND alternative therapies AND prescribed treatments; however, as noted in the US Institute of Medicine review,8 patients are often reluctant to divulge such information. Thus, physicians are encouraged to initiate AND maintain open communication about complementary AND alternative medicine.63 AND 64
