EUS - תוצאות מדאיגות

שלום רב! בבדיקה EUS IPMN (BD-IPMN or mixed IPMN) with 3mains cystic lesions in pancreas: -Bigger one located in uncinated process 4X2.6cm large containing a possible mural nodule 7X4mm large sticked to internal face of the cyst in its lower and right part. FNB taken throughout duodenal wall with a 19g needle (olimpus). Visquous liquid (as mucus) and some solid stuff were got and sent for cytology and pathology , and markers (CA19.9 CEA) amylase -another cyst in uncinated process 1cm large - a septaded cyst lesion in junction body neck , 15X14mm large No solid component was sen inside the 2 last cysts Mild dilatation of MPD up to 4.5mm in head with stenosis on its of echogenic finding in its lumen Normal looking papilla. But small lithiasis seen in distal CBD lumen 6 and 3mm large. Mild dilatation of MPD up to 8mm. Retracted gallbladder with thickened wall without clear lithiasis inside it Heterogenous fatty liver. CA-19.9 ירד מ66 ל 50 תוצאות ביופסיה: מעט תאי ציפוי עמודי מפריש ריר,בודדים וברצועות, במראה שיכול להתאים ל- IPMN בדיספלזיה קלה מה הטיפול המומלץ במקרה הזה? עד כמה זה מסוכן ? הרופא גסטרו שביצע את הבדיקה המליץ על הניתוח השאלה האם זה מוצדק במקרה הזה ומה אופציות נוספות . מדובר בבן 82 תודה רבה
